Please give the forms in at our receptionist at the school.

Find the entry form and school fees forms attached above. Download the document and complete the information on the document with a black pen. Please come to our school's office and hand in the document to our receptionist.

You must submit the following forms with the entry documents (This is extremely important):

The learner's birth certificate or ID Document.
Immunization card
Transfer card
Latest School Report card
ID copies of parents/guardian
Proof of residence/work address
Learner ID photo

If you do not have the necessary facilities to download the forms, come and visit the office and we will give it to you.
